Going Home
Planning for Your Discharge
You, your family, your physician and our hospital staff will plan your discharge. That planning could include care at home or in another setting after you leave the hospital. Or you might need special supplies and equipment in your home, or a referral to a public health department or a home care agency. You may choose any company or agency you like for those services. Your insurance company, Medicare or Medicaid may pay for home care services. Our staff can discuss financial arrangements with you.
We encourage you to ask any questions or express any concerns you or your family may have about your return home. Ask your nurse to contact Social Services for help in arranging for your post-hospital needs.
Test Results
If you have not received notification of your test results within one week, please contact your primary healthcare provider's office.
Transportation
It is easier and less stressful to return home from the hospital early in the day. You should make arrangements for your ride to arrive as soon after discharge as possible. Remember to check your room so you don't forget anything. Be sure to take home any valuables you may have placed in the hospital safe.
If you have no family member or friend to take you home and you need help with transportation arrangements, please tell your nurse or social worker.
