Transitional Care
You've had a hip replacement, cardiac bypass surgery, a stroke or maybe an accident. You no longer need acute hospital care, but your doctor says you need more rehabilitation before you go home. That is what we call our transitional care.
At Pine Medical Center, we offer fewer services – but outstanding care – in our hospital or long-term care facility while you're receiving rehabilitation in our transitional care program. And your costs will be less, too.
The main goal of transitional care is for you to become self-sufficient enough to return home.
You might have been hospitalized elsewhere, but with your physician's approval you can transfer to our medical center to receive the care you need to regain your strength and abilities. At Pine Medical Center, you'll be near your family and friends.
