Social Services

Our Social Services department is here to help you and your family. Our main function is to see that you or your loved one is safe after being discharged from Pine Medical Center. Our social worker connects patients with community services such as nursing homes, adult foster care and Meals on Wheels.

We make a discharge plan for every patient who leaves the hospital based on what the doctors, nurses and pharmacists say. Our social worker coordinates the plan. She also can help connect you with appropriate county resources.
